The transplant itself is only half the outcome. Graft survival, the quality of the healing scalp, and the patient's final density are shaped heavily by what happens in the ninety days after surgery. Why the post-op window matters

Newly transplanted follicular units depend on rapid revascularisation to survive. In the first days they are fragile, the recipient and donor areas are healing wounds, and the scalp barrier is compromised. Three things determine how well this window goes: protecting grafts from mechanical disturbance, keeping the healing environment clean and adequately moist, and supporting barrier recovery without irritating the tissue. A standardised protocol matters because variability is the enemy of consistent outcomes.


Phase 1 — Days 0 to 3: protect and stabilise

The priority is graft security; grafts are most vulnerable to dislodging in the first 72 hours. Minimise any contact or friction with the recipient area, manage expected mild oozing and crusting, control swelling, and keep the patient compliant with the no-touch rule.

Topical actives are kept minimal here. Aggressive products on day 1 do more harm than good — the goal is a calm, undisturbed healing field. Saline or the clinic's prescribed gentle mist is usually all that touches the recipient zone.


Phase 2 — Days 4 to 14: gentle cleansing and barrier support

Once grafts begin to anchor, the protocol shifts to careful cleansing (low-pressure, no rubbing, patting not wiping) and barrier support as the epithelium re-forms. Phase 4 — Days 31 to 90: regeneration and density support

From roughly one month onward, the focus turns from wound healing to growth support. The transplanted follicles are entering a new cycle, and surrounding native hair can be supported at the same time.
